RCA -- Root cause analysis
Yesterday there was a fire accident in the Carton Towers in the old Airport road in bangalore. A tragic incident where 9 persons died, and few injured. Its shown in TV that some people jumped from the upper floors. Sad.. :( ..
I see Corruption being the root cause of all these issues, Corruption in all levels, and lethargic mentality of the Officials to ignore the value of the life. How many times we have had fire accidents, for a year people and government remembers and then again starts behaving as usual, and blame the fate being the reasons for the Loss of lives.
The accident could have been averted --
- Had the Wires be of good quality.
- Had the work done was proper.
- Had the maintenance done properly.
- Had there been enough exit routes which were not locked for security reasons.
- Had there not been the idiotic people parking cars and bikes in road to see as if it was some show going on.
- Had there been some pubic sense while driving on the roads to allow the fire brigade to move.
- had there been the responsible officials who check the office building before giving the clearance.
- had there been adequate Ladders in City fire brigade to help people.
- had people enough knowledge of what to do in case of fire.
Every time something happens people put check points which are always manipulated and still we see the same things happening again and again. Its the government who puts us people in the unsafe places, and then come out after the loss announcing relief. It has to start expecting things rather than its too late..
Government needs vision, but it works in such an ad-hoc manner that they really work after disaster, and blame other. As long as it does not realize it its fault.. we will see lot more deaths lot more losses.. and we will feel pity and then continue doing things and contribute to new ways of disaster to come to us...
